Disparities in health outcomes are often the result of inequitable socio-economic circumstances. The PCC works to support all people in leading healthy lives with the recognition that people who experience racial, gender, and other inequities are likely to experience poorer health outcomes. This conference will explore the many different ways health inequities are apparent in our health care system and how to lift up opportunities to create more equitable, person-centered systems and culture of care delivery.
